Help - Extracting and collecting data from a Word Form Doc.

hello, work nfp organization work local school in community  and provides field trip opportunities students of grade levels.  i want create form fill out different schools information, data gets saved excel data sheet.  i going need 10-12 data fields can extract data form , have saved in excel file. examples:  organization,date of program, contact person, cell phone, email ,  program booked, time of program, cost, amount of students...

create these registration forms on 150 programs every year need have of saved 1 data sheet if @ possible.

have paper copy form have been using publisher 2010 , have created similar document in infoshare 2010 don't know created.

from you've posted, seems might better input data directly excel , use mailmerge, example, populate word (or publisher) documents. that's far easier implement , maintain trying go other way, mailmerge doesn't require programming.
